Enemy/items being in the wrong spot

Recently i've been experiencing enemys and items appearing in the wrong spot. What I mean by this is basically the enemy/item looks to be in a particular spot, then when you interact with it (shoot an arrow at the enemy or try to pick up the item), it will "teleport" to a different location (closeby, but totally different spot).
